Withdrawal form
If you booked a paid consultation as a consumer and want to withdraw, you can use this form. You do not have to: any other clear statement, by email or by post, is just as valid.
The cancellation and withdrawal terms you accepted when booking are on your appointment page, which you reach from the link in your confirmation.
